Article 28 Services
Since May 2000, HeartShare Wellness has been licensed by the New York State Department of Health under the NY Public Health Law as an Article 28 Diagnostic and Treatment Center specializing in services for individuals with intellectual and developmental disabilities.
Our Article 28 Services include:
- Internal medicine
- Pediatric, adolescent and adult psychiatry
- Podiatry
- Neurology
- Optometry
- Gynecology
- Restorative speech, occupational and physical therapies
No referral form is required for Article 28 services.
