Anyone with a developmental disability is eligible for services. Patients come through self referral, parents, service coordinators, physicians, other heath care professionals and government agencies concerned with the well-being of those with a developmental disability.
A referral form must be completed and signed to receive treatment at HeartShare Wellness.
This form includes authorization for tratment, release of information, financial agreement and Medicare benefits assingment for those enrolled in Medicare.
Individuals are seen in the clinic by appointment only. Please bring your Medicare/Medicadid or private insurance cards to each clinic appointment.
