In order for any patient to begin services at
Hearshare Wellness, LTD., it is necessary to complete and sign the referral form. This form includes authorization for tratment, release of information, financial agreement and Medicare benefits assingment for those enrolled in Medicare. Individuals are seen in the clinic by appointment only. Please bring your Medicare/Medicadid or private insurance cards to each clinic appointment.
