Psychological Evaluations
HeartShare Wellness psychologists conduct comprehensive psychological evaluations that begin with a clinical interview and then include any number of psychological tests (such as WAIS, Stanford-Binet, Bayley, Wechsler, etc.), depending on the individual's age and level of functioning. Testing generally takes several hours to complete and may require more than one appointment. The evaluation may also include interviews with significant others (such as parents, guardians, other caregivers, and MSCs), as well as a review of past records and relevant documents. A formal integrated written report of findings is provided along with recommendations based on those findings.
Sexuality Assessments
HeartShare Wellness's psychologists conduct comprehensive sexuality assessments using the SSKAAT-R in order to determine the individual's level of psychosexual awareness, understanding and knowledge. This tool assesses different sexuality sub-categories, including anatomy, intimacy, birth control, and sexually transmitted diseases. The SSKAAT-R is used primarily with individuals who are 18 years of age and older. A formal integrated written report of findings is provided along with recommendations based upon those findings.
Guardianship Determination
When a caregiver seeks to obtain legal guardianship of an individual, Heartshare Wellness will assist in completing the Affidavit (Certification) of Examining Physician or Licensed Psychologist. The Surrogates Court of the State of New York requires that a licensed psychologist and or physician complete the Affidavit (Certification) of Examining Physician or Licensed Psychologist attesting to his/her opinion that the individual is incapable of handling his/her medical affairs for the foreseeable future. The psychologist or physician may base his/her opinion on a medical examination, a recent psychological evaluation and an interview with the individual.
